INSURANCE INFORMATION: Our insurance begins where yours terminates. It is only valid when your policy has been extended to its limits. In the event you have no personal policy, our policy will provide you with coverage within its limits.
CONSENT FOR MEDICAL TREATMENT: As the parent or legal guardian of the above-named clubber, I hereby give consent for emergency medical care prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry. This care may be given under whatever conditions are necessary to preserve life, limb or well-being of my dependent.
PHOTO RELEASE CONSENT: Your child’s photo may be taken for inclusion in FBC publications or in local newspapers or magazine articles or letters relating to Awana activities. Please check below:
By submitting this form, I, the parent/guardian, of the registrant, a minor, agree that I and the registrant will abide by the rules of the Faith Baptist Church Awana Program and its affiliated organizations and authorize transportation by Faith Baptist Church as needed. Recognizing the possibility of physical injury associated with this activity, I hereby release, discharge and otherwise indemnify Faith Baptist Church Awana program and its affiliated organizations, their employees, teachers, and associated personnel against any claim by or on the registrant’s participation in the program except as covered by the program’s insurance.

Unfortunately, at this time, you have to pay for each registered child individually. Hopefully this will be updated in the near future. Also, you will be directed to paypal for payment.
